A new Diablo game means new demonic villains to fight. Lilith, often referred to as the Blessed Mother, is the antagonist in Diablo 4 after being summoned back to Sanctuary after centuries of banishment. However, despite being the game's main antagonist, there is a lot of lore surrounding Lilith from previous entries in the series, which can be confusing for newcomers.

As the main antagonist of the game, you'll be seeing a lot of her throughout the campaign. However, she'll often be one step ahead of you, maintaining a mysterious presence that makes learning about her difficult. Below, we'll go over everything we know about Lilith, including where she came from, what she's done, and more.

Who Is Lilith?

Lilith has a number of titles, including the Daughter of Hatred, Mother of Sanctuary, and Queen of the Succubi. Despite her first major appearance being in Diablo 4, she has done quite a bit behind the scenes throughout the Diablo world and story.

Lilith's Origins

Lilith is the daughter of Mephisto, who is the embodiment of Hatred and one of the three Prime Evils, along with Baal and Diablo. As the Daughter of Hatred, she inherited parts of this aspect, making her manipulative and vindictive. However, instead of serving the demons of the Burning Hells, she sought to rebel against her father.

Seeking an end to the Eternal Conflict, Lilith manipulated the addled mind of the archangel Inarius, who had fallen in battle. He soon fell in love with Lilith, and together they formed a plan to steal the Worldstone and use it to create their own new world.

Using the power of the Worldstone, Lilith and Inarius created Sanctuary, a world where angels and demons alike could find refuge from the Eternal Conflict.

Along with Sanctuary, Lilith and Inarius gave birth to the first Nephalem, beings that had the power of both angels and demons. The first Nephalem was Linarian, who later became the first Necromancer, Rathma.

For Inarius, simply escaping the Eternal Conflict was enough, but Lilith wanted more. She planned to use the Nephalem to finally win the Eternal Conflict. However, the strength of the Nephalem gave concern to both the followers of Lilith and Inarius.

Both Heaven and Hell invaded Sanctuary, leading to war and slaughter across the land. Fearing the extinction of her children, Lilith savagely fought back, killing armies of both angels and demons.

This caused Inarius to banish Lilith from Sanctuary into the Void, a place of endless darkness. Then, he used the power of the Worldstone to cause the power of Nephalem to decline over time, until eventually, they became the human race, mortals with significantly less power than their ancestors that made up the vast majority of the population of Sanctuary.

Lilith returns to Sanctuary in Diablo 4, depicted in the opening cutscene of the game. Three adventurers, led by a scholar, explore an ancient chamber, but are then ambushed by undead creatures.

Fleeing further into the ruins, they come to an altar. The scholar then ritually sacrifices the adventurers, using their blood to form a gateway through which Lilith passes through.

Now once again free in Sanctuary, Lilith roams the land, causing her corruption to spread and convert the people of Sanctuary into followers and cultists.

You play as the Wanderer, a traveler partially corrupted by Lilith's influence, allowing you to track her across the land and uncover her plots and schemes.
